GENERAL WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK, INDEMNITY AGREEMENT, AND MULTIMEDIA RELEASE

(AXE THROWING & RAGE ROOM ACTIVITIES)

TO: 102190362 Saskatchewan Ltd., its officers, directors, shareholders, employees, contractors, agents, affiliates, successors, assigns, legal representatives, and any person or entity acting under its direction or control (collectively, the "Released Parties")

WHEREAS the undersigned (the "Participant") desires to participate in axe throwing activities, rage room (smash room) activities, and/or any other events, programs, services, use of facilities, equipment, or premises offered by or on behalf of 102190362 Saskatchewan Ltd. (collectively, the "Activities");

NOW THEREFORE, in consideration of being permitted to participate in the Activities, the Participant agrees as follows:

1. RELEASE AND WAIVER OF LIABILITY

The Participant, on behalf of themselves and their heirs, executors, administrators, successors, and assigns, hereby irrevocably waives, releases, and discharges the Released Parties from any and all claims, demands, liabilities, damages, losses, costs, or expenses of any kind (including legal fees on a solicitor-client basis), arising out of or related to participation in the Activities, including but not limited to: personal injury or death; property loss or damage; any other loss or damage, howsoever caused. This release applies regardless of whether such claims arise from negligence, breach of duty, or otherwise, including under applicable legislation such as the Occupiers Liability Act.

2. ASSUMPTION OF RISKS

The Participant acknowledges that the Activities involve significant and inherent risks, dangers, and hazards, including but not limited to: throwing axes or similar objects; handling or breaking glass, ceramics, electronics, or other materials; flying debris, sharp fragments, and impact injuries; slips, trips, and falls; equipment malfunction or misuse; physical exertion, stress, or shock; negligence of other participants or staff. The Participant understands that these risks may result in serious injury, open wounds, permanent disability, worsening of existing medical conditions, or death. The Participant voluntarily accepts and fully assumes all such risks.

3. MEDICAL CONDITION AND FITNESS

The Participant confirms that they are physically and mentally capable of safely participating in the Activities. Participation is not recommended for individuals with conditions including but not limited to: asthma, epilepsy, or seizures; cardiovascular or respiratory conditions; hypertension; skeletal, joint, or ligament injuries; pregnancy; certain mental health conditions. The Participant agrees not to participate while under the influence of alcohol, drugs, or narcotics. The Released Parties reserve the right to deny participation to any individual suspected of impairment.

4. INDEMNIFICATION

The Participant agrees to indemnify and hold harmless the Released Parties from any and all claims brought against or incurred by the Released Parties arising from: the Participant's participation in the Activities; the Participant's actions or omissions; the actions of any guests or invitees of the Participant; any breach of this Agreement.

5. COMPLIANCE WITH RULES

The Participant agrees to follow all safety rules, instructions, and procedures provided by staff. The Participant will immediately report any unsafe conditions and acknowledges that failure to follow rules may result in removal without refund.

6. ARBITRATION

Any dispute arising out of or related to this Agreement shall be resolved by binding arbitration pursuant to the Arbitration Act (Saskatchewan). The arbitrator's decision shall be final and binding.

7. GOVERNING LAW AND VENUE

This Agreement shall be governed by the laws of the Province of Saskatchewan. Any legal proceedings shall be brought exclusively in Regina, Saskatchewan.

8. SEVERABILITY

If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

9. ENTIRE AGREEMENT

This Agreement constitutes the entire agreement between the parties and supersedes all prior communications or understandings.

10. MULTIMEDIA RELEASE

The Participant grants 102190362 Saskatchewan Ltd. a perpetual, worldwide, royalty-free license to photograph, record, and use their image, voice, and likeness for promotional, marketing, and commercial purposes in any media. The Participant waives any right to compensation and releases the Released Parties from any claims related to the use of such recordings, including claims of defamation, invasion of privacy, or misappropriation of likeness.

ACKNOWLEDGEMENT

I confirm that I have read and understood this Agreement in full. I acknowledge that I am waiving significant legal rights, including the right to sue, and I sign this Agreement voluntarily.
